1. Emergency Preparedness Plan

All IHC support workers must be prepared to respond to emergencies that may occur during a client's scheduled service visit. In any emergency situation, the worker's first priority is the safety and well-being of the client.

• Medical emergency: Call 911 immediately. Do not leave the client alone. Stay on the line with the dispatcher and follow their instructions. Notify IHC as soon as the client is stabilized or emergency services have arrived.
• Severe weather: Move the client to the safest interior room away from windows. Follow local emergency broadcasts. Do not leave until the situation is safe.
• Power outage: Ensure the client is safe and comfortable. If the client uses electrically powered medical equipment, call 911 if there is an immediate health risk and notify IHC.
• Reporting: All emergencies must be reported to IHC within 24 hours by calling (612) 900-2854 or emailing info@inthomecare.com. A written incident report must be completed within 24 hours.

2. Fire & Disaster Plan

Workers must be familiar with the layout of each client's home, including the location of exits, fire extinguishers, and emergency contacts posted in the home.

• In the event of fire: Assist the client to evacuate immediately using the nearest safe exit. Do not use elevators. Call 911 once outside. Do not re-enter the building.
• If evacuation is not possible: Close all doors between you and the fire. Signal from a window. Call 911 and advise them of your exact location.
• Natural disaster: Follow local emergency management instructions. Shelter-in-place or evacuate as directed by authorities. Notify IHC and emergency contacts listed in the client's care plan.
• After any disaster event: Document what occurred and report to IHC within 24 hours. IHC will coordinate follow-up care and any needed changes to the service schedule.

3. Hours of Work Policy

• Scheduled hours: Workers are assigned scheduled hours based on client authorization and availability. Schedules are managed through IHC's Billiyo scheduling system.
• Overtime: Overtime is hours worked over 40 in a workweek and is compensated at 1.5x the regular rate per Minnesota law. Overtime must be pre-approved by IHC management. Unauthorized overtime may result in disciplinary action.
• Split shifts: Workers may be assigned split shifts across multiple clients. Travel time between clients is not compensated unless arranged in advance.
• Minimum shift length: The minimum billable shift is 15 minutes. Workers must be present for the full authorized duration of each visit.
• Schedule changes: Requests for schedule changes must be submitted to Nadia Kewden at Nadia@inthomecare.com at least 48 hours in advance whenever possible.

4. EVV & Timecard Policy

Federal law (21st Century Cures Act) requires Electronic Visit Verification (EVV) for all Medicaid-funded personal care and home health services. IHC uses Billiyo for EVV, scheduling, and payroll.

• Clock-in: Workers must clock in at the start of every shift using the Billiyo app from the client's physical location. Clocking in before arriving at the client's home is prohibited and constitutes fraud.
• Clock-out: Workers must clock out at the end of every shift at the client's location. Services must not be documented beyond what was actually provided.
• Missed clock-in/out: If a worker forgets to clock in or out, they must notify IHC the same day. IHC may manually enter the time with supporting documentation. Repeated missed punches may result in disciplinary action.
• Accuracy: Falsification of time records or EVV data is grounds for immediate termination and may be reported to DHS as fraud.
• Payroll cycle: Wages are processed based on verified EVV records. Pay discrepancies must be reported to IHC within 5 business days of the pay date.

5. Return to Work Policy

• Illness: Workers who are ill must not report to a client's home. Contact IHC as early as possible so a replacement can be arranged. Workers with contagious illness must be symptom-free for at least 24 hours before returning.
• Injury: Work-related injuries must be reported to IHC immediately. Workers must not return to client assignments until they receive medical clearance and notify IHC in writing.
• Leave of absence: Workers returning from a planned or unplanned leave of absence must notify IHC at least 3 business days before their intended return. IHC will confirm schedule availability and complete any required re-onboarding steps.
• Background study renewal: Workers whose DHS background study clearance has expired may not return to client assignments until a new clearance is confirmed.

6. Professional Boundaries Policy

Support workers must maintain professional boundaries with clients and their families at all times. The worker-client relationship is a professional caregiving relationship — not a personal friendship or family relationship.

• Prohibited conduct: Accepting gifts, money, or items of value from clients or families; borrowing money; entering into financial or personal relationships; becoming romantically involved; sharing personal social media accounts; or visiting clients outside of scheduled shifts.
• Social media: Do not post images, videos, or identifying information about clients or their homes on any platform. This includes stories, reels, or private messages. Violations are grounds for immediate termination.
• Communication: Client and family contact information is provided for care coordination only. Do not contact clients for personal purposes outside of scheduled work.
• Confidentiality: All information about a client's health, living situation, and family is strictly confidential. Never discuss client information with anyone outside of IHC management.

7. Abuse Reporting & Mandated Reporter Policy

All IHC employees are mandated reporters under Minnesota law. This means you are legally required to report any suspected abuse, neglect, or financial exploitation of a vulnerable adult or child — immediately, without exception.

• What to report: Any suspected physical, emotional, sexual, or financial abuse; neglect; or maltreatment of a client, regardless of who the alleged perpetrator is (including other workers, family members, or IHC staff).
• How to report: Call the Minnesota Adult Abuse Reporting Center (MAARC) at 1-844-880-1574, available 24/7. If the client is in immediate danger, call 911 first.
• Notify IHC: After making a report to MAARC, notify IHC at (612) 900-2854 or info@inthomecare.com within 24 hours. IHC is also required to report and will follow up with DHS.
• No retaliation: IHC strictly prohibits retaliation against any employee who makes a good-faith abuse report. Failure to report suspected abuse is a violation of Minnesota law and IHC policy, and may result in termination.

8. HIPAA & Confidentiality Policy

IHC is a HIPAA-covered entity. All employee and support worker access to client health information (Protected Health Information / PHI) is strictly regulated.

• Access: Workers may only access client health information that is directly relevant to the care services they provide. Do not access any client files or records beyond what is needed for your shift.
• Disclosure: Never share, discuss, or disclose client health information with anyone other than IHC management and authorized care team members. This includes family members of the client, unless authorized in writing.
• Documentation: All service documentation must be completed accurately and submitted through authorized IHC channels only. Do not retain client records on personal devices.
• Breach: Any suspected or confirmed breach of client information must be reported to IHC immediately. IHC will follow HIPAA breach notification procedures as required by law.
• Violations: HIPAA violations may result in civil and criminal penalties under federal law, as well as immediate termination of employment.

9. No-Show / Call-Off Policy

Reliability is essential to client safety. Clients depend on their scheduled workers — a missed shift can have serious consequences for a vulnerable person.

• Call-off procedure: If you cannot make a scheduled shift, notify IHC as early as possible — at least 2 hours before your shift start time — by calling (612) 900-2854. Do not only text or email unless you receive confirmation from IHC staff.
• No-call no-show: Failing to report to a shift without notifying IHC is a serious violation. A first no-call no-show results in a written warning. A second may result in removal from the schedule. A third is grounds for termination.
• Pattern of absences: Frequent call-offs, even with notice, may result in schedule reduction or termination if the pattern cannot be resolved through accommodation.
• Emergency absences: IHC understands that genuine emergencies arise. Contact us as soon as you are safely able to. IHC will work with you in good faith on genuine emergency situations.

10. Social Media Policy

• Client privacy: Never post, share, or reference any client, their home, their family, or any information about their care on any social media platform — including Facebook, Instagram, TikTok, Snapchat, X (Twitter), or any other platform. This includes photos, videos, stories, and private messages shared with others.
• IHC representation: Do not represent yourself as speaking on behalf of IHC on social media without written authorization from IHC management.
• Professional conduct: Workers should be aware that social media activity that is threatening, harassing, discriminatory, or that reflects negatively on IHC or its clients may result in disciplinary action, even if posted on personal accounts outside of work hours.
• Violations: A single violation of client privacy on social media may result in immediate termination and could trigger a HIPAA investigation.

11. DHS Background Study Policy

All IHC employees and support workers must hold and maintain a current, cleared Minnesota DHS background study before providing any services to clients.

• Initial clearance: No employee may have unsupervised contact with clients until a cleared background study is received from DHS. IHC will initiate the background study process during onboarding.
• Disqualification: Certain criminal convictions disqualify individuals from working in home care under Minnesota Statute §245C. IHC follows all DHS disqualification guidelines and may not hire individuals with disqualifying records unless a set-aside has been granted by DHS.
• New arrests or convictions: Employees must disclose any new arrest, charge, or conviction to IHC within 48 hours of the event. Failure to disclose is grounds for termination.
• Renewal: Background study clearances are subject to DHS reconsideration at any time. IHC monitors clearance status and will notify workers of any change in their clearance status.

12. Incident Reporting Policy

Under Minnesota Statute §245D.06, IHC is required to report certain incidents to DHS and the client's case manager. All employees must be familiar with what constitutes a reportable incident and how to report it.

• Reportable incidents include: Client injury (accidental or otherwise), medication errors, falls, behavioral incidents, property damage, emergency hospitalizations, and any suspected abuse or neglect.
• Immediate notification: Notify IHC by phone at (612) 900-2854 as soon as the client is safe. Do not wait until the end of your shift.
• Written report: A written incident report must be submitted to IHC within 24 hours of the event. IHC will provide the required form. IHC then has specific reporting timelines to DHS and the case manager depending on the severity of the incident.
• Documentation: Write down exactly what you observed — not opinions or conclusions. Record the time, what happened, how the client responded, and what actions you took.

What is EVV?

Electronic Visit Verification (EVV) is a federal and state requirement under Minnesota Statute §256B.073 and the 21st Century Cures Act. Every IHC support worker must clock in and out electronically for every shift — from the client's home address. IHC uses Billiyo as its EVV, scheduling, and billing platform.

You Have the Right to Choose

Under the CFSS Agency Model and PCA programs, you have the right to choose who provides your care — including a family member or someone you already trust. IHC supports your choice and will work with you to enroll your preferred worker as quickly as possible.

Can a Family Member Be My Support Worker?

Yes — in most cases. Under CFSS and PCA, a family member (spouse, sibling, adult child, relative, or close friend) can be enrolled as your paid support worker through IHC. IHC becomes the legal employer, handles payroll, taxes, and DHS compliance — your family member just focuses on caring for you.

Who Cannot Be Your Worker

• The owner or operator of the home care agency (IHC)
• Someone who has a disqualifying DHS background study result
• Someone under age 16 (or 18 for most 245D positions)
• Someone you are currently paying privately for the same services

How to Enroll Your Chosen Worker

• Contact IHC and let us know who you would like as your worker
• Your worker completes IHC's employee application
• IHC initiates their DHS background study
• Once cleared, your worker completes orientation and begins getting paid

The whole process typically takes 1–2 weeks depending on background study processing time.

Can I Change My Worker?

Yes — at any time, for any reason. You can request a change of worker by contacting IHC. You will not lose your authorized hours or service plan by making a change. Your right to choose your caregiver is protected under both CFSS (§256B.85) and Minnesota client rights law.

• Support Worker — CFSS & PCA
  Provide personal care assistance including bathing, dressing, mobility support, meal preparation, and community access for clients under the CFSS Agency Model and PCA programs. The most common position at IHC. CFSS positions are open to applicants age 16 and older.
• Personal Support Specialist — 245D
  Provide one-on-one support and supervision for individuals on 245D waiver programs. Focus on community integration, skill development, and daily living assistance.
• Night Supervisor
  Provide overnight awake supervision for clients who need safety monitoring, behavioral support, or nighttime personal care. Overnight shifts — ideal for those who prefer evening or overnight availability.
• Homemaking Aide

Our Licensing & Compliance

IHC is fully licensed by the Minnesota Department of Human Services under multiple frameworks:

• CFSS Agency Model — Licensed under Minnesota Statute §256B.85 to provide Community First Services and Supports under the Agency Model. IHC is the employer of record for all support workers.

Our Multilingual Commitment

Our Intake Process

IHC partners with C.A. Ellis & Associates, LLC, an independent third-party organization, to conduct all client intake meetings and 245D documentation. This ensures that intake is conducted by a neutral, qualified professional — protecting clients' rights and ensuring all documentation meets DHS standards from day one. Once intake is complete and all documents are signed, IHC matches the client with a qualified support worker and services begin.

International Home Care Inc. (IHC) is committed to protecting the privacy and confidentiality of all individuals we serve, including clients, families, employees, and website visitors. HIPAA & Health Information

IHC is a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). All protected health information (PHI) — including medical history, service records, diagnoses, and care plans — is handled in strict accordance with HIPAA regulations and Minnesota state health privacy laws. PHI is collected only for treatment, payment, and healthcare operations as permitted by law. Clients receive a separate Notice of Privacy Practices at intake.

Information We Collect

Depending on the context, IHC may collect the following categories of information:

• Client information: Name, date of birth, address, phone, email, Medicaid ID, diagnosis, service authorization details, and care-related documentation required by DHS and your county or MCO.
• Employee & applicant information: Name, contact information, employment history, background study results, training records, and payroll information required to operate as a licensed employer.
• Website visitor information: When you use our website contact, intake, referral, or appointment forms, we collect the information you submit. We do not use tracking pixels, third-party advertising cookies, or behavioral analytics.
• Communication records: Emails, phone call notes, and chat messages sent through our website for purposes of responding to your inquiry.

How We Use Your Information

• To enroll and provide authorized home care services to clients
• To verify insurance and waiver eligibility with your county or MCO
• To complete DHS-required documentation under CFSS, PCA, and 245D regulations
• To bill Medicaid and managed care organizations for authorized services

Who We Share Information With

IHC does not sell, rent, or trade personal information to any third party for marketing purposes. We share information only when necessary and permitted by law, including with:

• Your county case manager or MCO — to coordinate service authorization, referrals, and care planning
• Minnesota DHS — for licensing, auditing, and regulatory compliance purposes
• C.A. Ellis & Associates, LLC — our independent third-party intake coordinator who conducts 245D intake meetings and documentation on IHC's behalf

• Background study vendors — as required by Minnesota DHS for employee clearance
• Legal or regulatory authorities — only when required by law, court order, or to protect the safety of clients or staff

Electronic Visit Verification (EVV)

Federal law (the 21st Century Cures Act) requires that all Medicaid personal care and home health services use Electronic Visit Verification. IHC uses Billiyo to record the time, location, and type of service delivered at each visit. EVV data is submitted to the Minnesota Department of Human Services as required. Clients are informed of EVV requirements at intake.

Website & Data Security

Our website is hosted on Netlify and uses HTTPS encryption for all data transmission. Forms submitted through our website are processed by FormSubmit and delivered to our secure business email. We do not store form submissions on our web server. We do not use Google Analytics, Facebook Pixel, or any third-party advertising trackers on this website.

IHC maintains physical, electronic, and administrative safeguards to protect personal information from unauthorized access, disclosure, or misuse in compliance with HIPAA Security Rule requirements.

Your Rights

• Access: You have the right to request a copy of the personal information IHC holds about you.
• Correction: You may request that inaccurate or incomplete information be corrected.
• Restriction: You may request that we limit certain uses or disclosures of your health information.
• Accounting of disclosures: You may request a list of disclosures IHC has made of your health information.
• Complaint: You may file a complaint with IHC or with the U.S. Department of Health and Human Services Office for Civil Rights if you believe your privacy rights have been violated. IHC will not retaliate against you for filing a complaint.

Retention of Records

IHC retains client service records for a minimum of five years following the end of services, or longer as required by DHS licensing rules and Minnesota law. Employee records are retained for a minimum of three years following separation. Website inquiry records are retained only as long as necessary to respond to and resolve the inquiry.

Harassment-Free Workplace

IHC strictly prohibits workplace harassment of any kind, including sexual harassment, racial harassment, and any other form of hostile or offensive conduct based on a protected characteristic. Harassment includes unwelcome verbal comments, physical conduct, written communication, or any behavior that creates an intimidating, hostile, or offensive work environment.

What is CFSS?

Community First Services and Supports (CFSS) replaced Minnesota's traditional PCA program. It gives individuals with disabilities greater control and flexibility over their own care. IHC operates under the Agency Model — we are the legal employer of your support workers. We hire, train, schedule, pay wages, handle taxes, conduct background checks, and ensure every worker meets DHS standards. You remain the director of your own daily life.

Night Supervision provides an awake caregiver present overnight for individuals who need safety monitoring, emergency response, or assistance during nighttime hours — critical for individuals at risk of falls, seizures, or behavioral incidents at night.

The Caregiver Living Expense option allows a live-in caregiver to reside in the home and be available overnight, with compensation for room and board as part of the arrangement.

What is 245D?

Minnesota Statute 245D governs Home and Community-Based Services for people with developmental disabilities, physical disabilities, mental illness, or traumatic brain injury. IHC is a licensed 245D provider, meeting all state requirements for training, documentation, rights protection, and service delivery.

Every 245D client receives a written Individualized Service Plan (ISP), full rights disclosure at intake, 45/60-day follow-up reviews, and annual reviews — all fully documented per DHS requirements.

In-Home Support Services (IHS) Without Training provides personal care and daily living support for individuals who do not require their support worker to have specialized training. Services are provided in the participant's home and community.

• Community access and transportation assistance
