Services Cannot Begin Without a Completed Intake

IHC cannot start any new services until a client intake has been completed. Case managers do not schedule the intake. After you submit a referral, IHC coordinates with C.A. Ellis & Associates, LLC — an independent third-party that conducts all intakes and 245D documentation — to schedule the meeting and send invitations to you and the client. Intake meeting invitations are sent from scheduler@caellisassociates.com.

How do I get started with IHC services? +
Fill out our online intake form or call (612) 900-2854. Our team contacts you within one business day to verify authorization and enroll you. If you don't have authorization yet, we can help guide you through the process.
Do I have to pay for services? +
No. All IHC services are billed directly to Medicaid. You are never charged out of pocket for authorized services. IHC cannot ask for payment or deposits of any kind.
What is the difference between CFSS and PCA? +
Both provide personal care funded by Medicaid. CFSS gives you more control — you direct your care and IHC employs your worker. PCA is the traditional program. Your county case manager can help determine which is right for you.
Can I choose my own support worker? +
Yes. You can request a worker who speaks your language, is a specific gender, or has relevant experience. You can request a worker change at any time. Family members may also serve as your support worker in many cases.
What counties does IHC serve? +
IHC serves Hennepin, Ramsey, Anoka, Dakota, Washington, Scott, Carver, Wright, Sherburne, Isanti, Chisago, Stearns, Rice, Goodhue, Le Sueur, and Mille Lacs counties. Call (612) 900-2854 to confirm your area.
Does IHC serve clients in other languages? +
Yes. IHC provides services in English, Somali, Oromo, Amharic, Arabic, and Spanish at no cost under Title VI. Call (612) 900-2854 and we will connect you with a staff member who speaks your language.
What if my worker does not show up? +
Call IHC immediately at (612) 900-2854. We will arrange a replacement worker and you will not lose your authorized hours. You have the right to reliable services every scheduled visit.
How do I apply for a job at IHC? +
Apply using our online Employee Application on this website. CFSS Support Worker positions are open to applicants age 16 and older. All other positions require age 18+. A cleared DHS background study and orientation training are required before starting. Bilingual candidates strongly encouraged. Contact: Nadia@inthomecare.com
What is EVV and why does my worker clock in from my home? +
EVV (Electronic Visit Verification) is required by federal law for all Medicaid personal care services. Your worker must clock in and out from your physical location every visit. Contact IHC if a worker clocks in elsewhere.
What waivers and insurance does IHC accept? +
IHC accepts Medical Assistance and all major waiver programs including DD, BI, CADI, CAC, and Elderly Waiver. We work with UCare, Hennepin Health, HealthPartners, BluePlus, Medica, MSHO, and Itasca Medical.
